The patients with body mass index of 40 and higher are evaluated in “Morbidly Obese” class. Since obesity is a fatal danger for these people, various surgical and medical methods are applied for weight loss. Especially, after “sleeve gastrectomy”, the patients experience a weight loss between 60-70 kilograms and skin excesses occur on every area of the body after such a weight and volume loss.
In any case, after excessive weight loss, the surgical treatment for the sagging and drooping of skin on the body is called “Post Bariatric Surgery”. Belt lipectomy, brachioplasty, thigh and leg reduction, chest reduction is included in “Post Bariatric Surgery”.
What is Belt Lipectomy?
The visible sagging and skin excess occurring on the body after excessive weight loss cause important problems. Skin sags on the belly, the back, tights, sides and back waist sections. The surgical treatment of such sagging occurring on the chest is ‘belt lipectomy’. The belt lipectomystretches the front face of belly and the back to the lower section and gives volume to the buttocks while lifting them with the extraction of sagging and excessive skin on the waist.
What is Thigh Reduction?
The thigh skin may sag to sides and downwards depending on the loss of skin elasticity and excessive weight loss. The ‘thigh reduction surgery’ is applied to the patients for removing the sagging. The thigh reduction surgery may be conducted along with liposuction for thinning the legs.
What is Arm Reduction?
The skin on the arms may sag downwards and sideward depending on the on the loss of skin elasticity and excessive weight loss. The ‘arm reduction surgery’ is applied to the patients for removing the sagging. The arm reduction surgery may be conducted along with liposuction for thinning the legs.
Post Bariatric Surgeries
Belt Lipectomy Surgery
The belt lipectomy is conducted with general anesthesia and approximately takes 4 hours. Firstly, the skin and subdermal tissue excessive on waist and back is removed. If reduction is visible on this phase, the tissues planned to be taken from the skin area are embed under the waist and a visible volume increment may be obtained. Thanks to this, the patient will have oval and voluminous buttocks. The patient lies in supine position on the following phase and a normal abdominoplasty surgery is conducted. Variants may be added to this surgery depending on the skin sagging on the body and thighs.
Recovery Period after Belt Lipectomy
The patient shall be observed in the hospital for 3 days after the surgery. The patient may move and satisfy his/her needs on the early period. It is suggested that the patient shall use a special bandage for 3 weeks and shaping corsets for 6 weeks after the surgery. Having a high protein diet improves the recovery period visibly on the recovery period. The marks of the cuts winds around the waist like a belt and are left under the underwear. With good care, an appearance which would not disturb the patient may be obtained.
Thigh Reduction Surgery and its Recovery Period
The thigh reduction surgery is conducted with general anesthesia and approximately takes 3 hours. In this operation, the excessive skin and subdermal tissues are extracted with the cuts extending through waist line at the inner side of the thigh. Therefore, the wrinkles on sides and sagging to downward are eliminated. The patient is observed for 2 days in the hospital. It is suggested that the patient shall use corsets shaped as tights for 4-5 weeks after being discharged from the hospital. The mark treatment must be conducted during the recovery period.
Arm Reduction Surgery and its Recovery Period
The thigh reduction surgery is conducted with general anesthesia and approximately takes 1.5 hour. In this operation, the excessive skin and subdermal tissues are extracted with the cuts extending through the arm pit line and at the inner side of the arm. Therefore, the wrinkles on sides and sagging to downward are eliminated. The patient is observed for 1 days in the hospital. It is suggested that the patient shall use armlets extending from shoulder to elbow for 4-6 weeks after being discharged from the hospital. The mark treatment must be conducted during the recovery period.
Matters To Be Considered for Post Bariatric Surgery
Most of the post bariatric patients are patients who had slevegastrectomy surgeries and lost excessive amount of weight. Having a stomach which cannot fulfill its physiological duties completely makes these patients and post bariatric surgery important.
1- The body counter surgeries must be conducted in séances for post bariatric surgery. The order and combinations of the surgeries are of importance.
2- The elasticity of the skin is very bad for post bariatric patients. Therefore, a tissue which you think is very stretched during the surgery may sag again in a very short time. Therefore, the surgery séances shall be planned with consideration that the next surgery would allow the revision of the previous surgery. For example, the abdominoplasty and mastopexy is not conducted in the same séance.
3- The healing of wounds is very problematic since the post bariatric patient feeds in much unorganized way. Therefore, in order to have full recovery during the surgeries, various agents, vitamins and protein content may be given.
4- In order to plan the surgeries of post bariatric surgery patients, the patients must have kept their weight same for at least 6 months. In case that the patient lose weight after the surgery, the skin will sag in a very fast fashion.
